Office Policies

The following information is to ensure a smooth transition into our practice.

Please arrive 5-10 minutes prior to your appointment time. Arriving late for your appointment may lead to the appointment being rescheduled. If you have medical insurance, please bring all current insurance cards. Copayments will be collected at the time of service. For self-pay patients, payment in full at the time of service is required unless a payment arrangement has been made at the time of check-in. We accept all major credit cards. Preventative visits are designed to identify risks and the best ways to stay healthy. Your insurance plan may cover a preventative visit at 100%. However, when asking your provider about specific health concerns, receiving a new diagnosis or discussing chronic conditions, your insurance company may apply a copay, coinsurance or deductible. Please verify plan limitations with your carrier. We are a Practice that believes vaccination is one of the most important things that can be done to keep children healthy. We expect to fully vaccinate each of our patients and look forward to working with you and your child.

Insurance and Billing

We accept most major insurance plans and will bill your insurance company for services provided.

Medical Records

Your child's medical records are confidential and will not be released without your consent.
