If you have medical insurance, please bring all current insurance cards. Copayments will be collected at the time of service. For self-pay patients, payment in full at the time of service is required unless a payment arrangement has been made at the time of check-in. We accept all major credit cards. Preventative visits are designed to identify risks and the best ways to stay healthy. Your insurance plan may cover a preventative visit at 100%. However, when asking your provider about specific health concerns, receiving a new diagnosis or discussing chronic conditions, your insurance company may apply a copay, coinsurance or deductible. Please verify plan limitations with your carrier. We are a Practice that believes vaccination is one of the most important things that can be done to keep children healthy.
